What Is Coccydynia?

Coccydynia ispain focused at the coccyx — the small, curved bone at the very base of the spine, commonly called the tailbone. It is often described as a sharp, aching pain that is dramatically worse when sitting — particularly on hard surfaces — and may radiate to the buttocks or lower back. Although not dangerous, coccydynia can be severely limiting, affecting the ability to sit comfortably, drive, or work at a desk. What Causes Tailbone Pain?

  • Directtrauma — falling ona hardsurface (most common cause)
  • Childbirth — the coccyx can be injured during a difficult delivery
  • Repetitive strain — prolonged cycling, rowing, or sitting on hard surfaces
  • Hypermobile coccyx — excessive movement of the coccyx
  • Coccyx spurs or degenerative changes
  • Infection or tumour in the coccygeal area (rare)
  • Idiopathic — no identifiable cause in some cases

Caudal Epidural Injection
A precisely guided injection of steroid and local anaesthetic into the caudal space (just above the coccyx) reduces inflammation around the coccygeal nerves. Often provides dramatic relief — sometimes after just one injection.

Ganglion Impar Block
The ganglion impar is a sympathetic ganglion located just in front of the coccyx. An injection to block this ganglion is one of the most effective treatments for coccydynia and perineal pain. Performed under fluoroscopic guidance.

Local Steroid Injection
A direct injection of corticosteroid and local anaesthetic around the coccyx reduces inflammation and pain. Typically performed under ultrasound guidance.

Pulsed Radiofrequency of the Coccyx Nerves
For persistent coccydynia that does not respond to injections, pulsed RFA of
coccygeal nerves provides longer-term relief.

Physiotherapy
Specific stretches targeting the coccyx and pelvic floor, along with sitting posture modifications (using a coccyx cutout cushion), complement interventional treatment.
